Privacy Policy

Shining Star Clinic LLC
Effective date: October 15, 2026

About this policy

This policy explains how Shining Star Clinic LLC handles information from visitors to shiningstarclinic.com and start.shiningstarclinic.com, people who contact us online, and adults who submit our Facebook or Instagram lead forms. It also explains cookies and similar website technology.

Our callback forms are for an adult parent, caregiver, or other adult making an inquiry. Please do not enter a child’s medical history, diagnosis, insurance member number, Social Security number, or medical records in a callback form or social-media message. Contact our intake team for the appropriate process for clinical information.

This website policy is separate from a HIPAA Notice of Privacy Practices and does not limit your rights regarding protected health information. Information connected to a request for care may receive additional protection under applicable law. Contact the clinic with questions about patient privacy.

Information we receive

  • Callback requests: your first and last name or full name, phone number, email address, city, preferred callback time, and the contact permission you provide. The callback form at start.shiningstarclinic.com requires these fields.
  • Other inquiries: information you choose to include in a contact form, email, phone conversation, or message. The main website also has intake forms that request information about a child and care needs. Separate intake or employment forms can request additional information for their stated purpose.
  • Meta lead forms: the contact information and answers you submit, along with information associated with the submission, such as its date, form, and advertisement. Meta may prefill fields from your profile. Review them before submitting.
  • Technical information: our website, hosting, security, and embedded-service providers can receive an IP address, browser or device information, requested page, request time, and related connection information. Cookies and interaction tools can collect additional information described below.

How we use information

We use inquiry information to respond, discuss services and next steps, send a request acknowledgment, manage follow-up, maintain appropriate business records, and protect our forms from spam or misuse. A callback preference is not a booked appointment. Submitting an inquiry does not guarantee service availability, insurance coverage, or admission.

The callback forms request contact about your inquiry. They are not a newsletter or unrelated promotional-message signup. You can ask us to stop inquiry follow-up or change your contact preferences using the contact details below.

Who receives information

Clinic staff handling your request and the service providers involved in operating the website, processing forms, hosting, delivering email, security, or technical support can receive information needed for those activities.

The website callback service uses Vercel for hosting, the clinic’s WordPress form system for staff notifications, the clinic’s email systems, and Resend for email acknowledgments. Submitting a Meta Instant Form sends your answers to Meta and makes them available to Shining Star Clinic through Meta’s business tools. Meta also handles information under its own Privacy Policy.

Website tools described below can receive technical or interaction information directly from your browser. We may also disclose information when required by law or as otherwise permitted by applicable law to address security incidents, protect legal rights, or respond to your request.

Cookies and similar technology

Cookies are small pieces of information stored in your browser. Websites also use browser storage, tags, and embedded services. These tools can remember settings, protect forms, measure visits, or support advertising. Their purposes differ.

Website or tool What it does
Callback page
start.shiningstarclinic.com
The current callback page does not load Google Tag Manager, Google Analytics, Meta Pixel, Microsoft Clarity, or Lucky Orange. It uses Google reCAPTCHA for spam protection. reCAPTCHA uses a security cookie and processes browser/device signals to help detect automated abuse. See Google’s Privacy Policy and Terms of Service.
Main website
shiningstarclinic.com
Google Tag Manager, Google Analytics, Microsoft Clarity, Lucky Orange, and advertising tracking are currently disabled on the main website. Complianz stores first-party cookies to remember your privacy choices. The embedded Google Maps feature is optional and remains blocked until you allow it. Google reCAPTCHA protects forms from spam, and GTranslate provides translation functionality. These functional services can receive technical information and use cookies or browser storage. Please avoid entering medical information into general inquiry fields.

You can block or delete cookies and manage third-party cookies in your browser. These settings apply to that browser and device and may affect maps, translation, or form verification. You can call us instead of using an online form. Meta’s privacy and advertising controls apply to your activity within Meta’s services.

The main website does not show an automatic cookie pop-up or a floating privacy button. Optional maps stay blocked until you choose to allow them. To allow the map or withdraw your choice later, use the Cookie preferences controls below. Essential website functions and form spam protection remain available. Your choice applies to that browser and device. Blocking future loading cannot undo information already received by a provider.

This policy explains data use; reading it is not consent to optional tracking. A contact-permission checkbox applies to your inquiry and does not authorize advertising trackers to receive protected health information. Browser controls do not stop every type of data collection.

Records, security, and your requests

Inquiry information can remain in clinic records, email accounts, and service-provider systems after a callback. Retention depends on the type of record, the follow-up needed, and applicable legal or recordkeeping requirements. Information that becomes part of a clinical record is subject to the requirements that apply to that record.

Our callback page uses an encrypted HTTPS connection and spam protection. No website, email system, or transmission method can promise complete security. Please avoid sending medical records or other sensitive documents by ordinary email or social-media message.

You can ask to access, correct, or delete your inquiry information, or ask us to stop contacting you about an inquiry. Depending on the laws that apply, you may have additional rights to a copy of your information or to restrict or opt out of certain uses. We may need to verify your identity, and some records may need to be retained. Contact us if you have concerns about how a privacy request was handled. You may also contact the Maryland Office of the Attorney General about applicable privacy rights.

Children and external services

Our public inquiry pages and social lead forms are intended for adults, even though the clinic serves children. A parent or authorized caregiver should contact us. If a child submitted personal information directly, please contact us so we can review and address it. Clinical intake information about a child follows the clinic’s intake and patient-privacy processes.

Links and embedded services operated by other organizations have their own privacy practices. This policy describes the clinic’s handling of information and does not control those organizations.
